About Us
The Eastern DHC Express provides district-based primary healthcare services through health promotion, diseases prevention, health assessment & consultation and chronic diseases management (e.g. hypertension, diabetes and musculoskeletal disorders etc.) to enhance public awareness of self-management of health. The Eastern DHC Express helps strengthen the involvement of individuals and communities, plans and coordinates among various medical and social sectors and aims to encourage the public to take precautionary measures against chronic diseases, enhance their capability in self‑care and home care, and reduce the demand for hospitalisation.
Health Bureau awarded the 3-year operation service contract of Eastern DHC Express to The Hong Kong Society for Rehabilitation.

Our Services
The Eastern DHC Express provides three levels of disease prevention services. Residents of or Working in Eastern District can register with their proof of address / working address and identification as members and participate in the three-tier prevention project:
